Photocopy of ID card (KTP) TOEFL score 400 3 pieces of 4×6 photos Curriculum Vitae Statement letter of being able to follow international class rules parent/guardian’s statement All files are put in a blue folder and submitted to the academic of Bachelor of Nursing Program, Faculty of Nursing on working days.
